Dear Sir (and, Madam),

To the best of my knowledge I am the sole author of the work in question, "A 
Starting Point", although it is obvious that there are appropriations from 
other authors -- Wallace Stevens and Jorie Graham perhaps being the most 
obvious. Again, to the best of my knowledge, it has not appeared anywhere in 
print (certainly, not with my permission).

Although I am uncertain as to the proper format required for your particular 
institution, an appropriate citation might look like the following.

It should be understood, of course, that S.Rush, or Serotonin Rush is a nom de 
guerre -- but what I was fighting or why has escaped me. Nonetheless, I have 
decided to keep the name. Perhaps to remind me of a more noble, less innocent 
time.

From personal experience I know that completing a doctoral dissertation can be 
extremely trying. I wish you both the best of luck, health, and success in 
your future endeavors.
